This registration will allow you to provide contact information, sign consents, and submit payment. Please read through each page carefully to ensure that all information is accurately provided. 1st - 12th graders who meet the following requirements:
FMLA follows all THSLL, DFWL, TGHSLL, and TGYLL boundary rules. Please e-mail the FMLA Board at email@example.com with any eligibility questions.
Payment Plan Available:
You can either pay in full or in four payments during the season.
Multiple Child Discount: If you are registering more than one child, in order for the registration system to recognize that your family is registering multiple players and receive the family discount (10%), please register all of your players in one session starting with the highest level player (ex. Varsity, JV, Youth). On the 'Review Page', there is a link near the bottom to "Register Another Player", please click on this link to ensure that all of your family's players are registered within the same session. Failure to do so will result in a forfeiture of your family's discounted rates.
Volunteer Discounts: If you are planning on serving as a head coach, assistant coach, or specified committee member and have confirmed that your service qualifies for a discount, please contact Matt McQuillen to obtain a discount code to use towards this registration before registering.
Should there be a situation where your child wants to play, but it is a financial burden, please contact the FMLA Treasurer for information on how we can help. All of these matters are handled confidentially.
Cancellations before Midnight August 30, 2019 will receive a 100% registration fee refund - cancellations that take place on or after August 30, 2019 will be reviewed and decided by the FMLA Board. There are no refunds for purchases uniforms or equipment.

Each player must have an active US Lacrosse membership to play lacrosse with FMLA - a player's US Lacrosse membership must remain active throughout the entire season, thus it is up to the parent to renew their child's US Lacrosse membership if set to expire prior to the conclusion of the season. The annual US Lacrosse membership fee is $30 for Youth players and $35 for High School players - please visit the US Lacrosse Membership FAQ page for additional information.
Step 1: Obtain current US Lacrosse Membership
IF YOU DO NOT HAVE A CURRENT US LACROSSE MEMBERSHIP: CLICK HERE to go to US Lacrosse Registration and register your player(s). Be sure to PRINT and SAVE their confirmation code(s). Remember to register EACH of your players with US Lacrosse. Each player must have their own unique US Lacrosse membership confirmation number.

Step 2: Return to this registration and enter the US Lacrosse Confirmation Code that you received in 'Step 1'
Note: If having issues entering your child's US Lacrosse membership number, try only need the enter the last seven digits of the number (all the numbers after the zeros) when beginning the registration process.
